In order to attend the event, each Member, Escort and their Parent must acknowledge the Rules, Consent, and Waiver of Liability Agreement. Registration to the Houston Girls Cotillion, Class of 2021 Junior Cotillion Dance, January 19, 2020 is not complete until the Agreement, signed by each Member, Parent, Escort and Escort's Parent is received by the Houston Girls Cotillion, Class of 2021 Executive Board. All Agreements must be on file at the event in order to be admitted to the event.

This Agreement shall be effectuated by signing and acknowledging the Agreement, after full review of its terms and granting full consent to same. Each minor Member and her Escort must consent and agree to all rules by signing this form, and each Parent (for both Member and Escort) must also consent to all rules and release of liability by signing this form. Minors may not consent on behalf of their Parents.

Member and Parent please complete the Member Consent, and Escort and Parent please complete the Escort Consent.

If Member changes her escort after January 7, 2020, a new Agreement must be executed by new Escort and Parent.

All agreements are due on or before Tuesday, January 7, 2020.

Registration / Refund Policy
Registration opens on Sunday, October 20, 2019, 3:00 p.m. - $350.
Late registration begins October 26, 2019 - $375.
All registration will close November 3, 2019 or sooner if we reach capacity of registered members before this date.
There are NO REFUNDS ISSUED for any reason at any time.
